Copies of the following documents must be attached:

WorkSafeBC clearance letter from all contractors and sub-trades. The WorkSafeBC clearance letter must be addressed to the owner who is hiring the contractor and state that the contractors account is Active and in Good Standing.

Proof of liability insurance showing the amount of liability your contractor carries. Please ensure contractors name and dates of coverage are visible.

I/we acknowledge that I have read and understand the Terms and Conditions set out in Schedule 'A' and agree that any alterations will be performed in accordance with those Terms and Conditions. I/we further agrees to comply with any direction by the Board of Directors regarding the manner in which the work is carried out.
